Abstract
Chronic diseases are a major current health problem and burden on the health system of many countries. If interventions are not developed and applied at the governmental level, the burden of chronic diseases will continue to increase in the Australian community. These diseases directly affect not only people with the diseases but other individuals in their near environment. In Australia, these chronic diseases include but are not excluded to: ischemic heart disease (IHD), chronic obstructive pulmonary disease (COPD), asthma, diabetes type 2, hypertension and chronic kidney disease (CKD).
