Abstract
Self-management, self-care or self-monitoring are terms often used by nurses in their practice. But do we often stop and think about what it means to our patients? Are our patients suitably prepared to self-manage their health condition in the home setting? Or do we send them home with superfluous brochures and resources, assuming all will be ok? Are our patients well-equipped to navigate a complex health system for information that would support them self-manage their condition?
