Abstract

Background to the study: Aboriginal women repeatedly request different type of service; 2006: Workshop to explore issues with current model of service provision; The key issues identified as the 4 C's: Choice, Coordination, Communication, Continuity; Initial Patient Journey models developed: Multiple discontinuities in service identified that warranted further research.
