Abstract
![CDATA[Reducing the incidence of inappropriate and often deleterious interventions that elderly people are subjected to in the last months of life is arguably the most important challenge for contemporary healthcare delivery. A new approach has been developed that simultaneously identifies the elderly frail near the end of life and empowers them to establish their own goals of care. The system is currently being evaluated, and strategies are in place to implement the new approach on a wide scale. The approach will shed new light on the way we currently view chronic conditions in the elderly frail as treatable and amenable to acute hospital management. Such a radical change in approach will need to occur in conjunction with open and vigorous dialogue between multiple stakeholder groups.]]
