Abstract
Why do people with type 1 diabetes have such difficulty achieving euglycaemia? An inverse association exists between reductions in hyperglycaemia and increasing hypoglycaemia: as patients approach euglycaemia, the key factor preventing further improvement is hypoglycaemia. Even mild hypoglycaemia is associated with reductions in quality of life. Management of glycaemia in type 1 diabetes therefore entails more than preventing increases in glucose and correcting hyperglycaemia promptly when it occurs; it is also critical to prevent hypoglycaemia.
