    Abstract

    ![CDATA[The key to many healthcare improvement projects is redesigning the system of care to be truly patient centric. Whilst much of the literature on value stream mapping depicts only a strong spotlight on the process flow, there is no consistent approach to focusing staff on patient-centric re-designs of the patient journey that also includes investigating patient movements, staff roles, information and technology, clinical guidelines and policies, patient needs and measurements. The Essomenic modelling technique allows for a comprehensive patient centric approach to redesign. The modelling technique includes a visualisation of interactions between patients and the system of care and has been proven to be successful in solving existing problems. In addition, it has delivered innovations which were readily adopted by the persons involved through their engagement and active learning.]]
