Warping of the levator hiatus : how significant is it?

Abstract

The levator hiatus is the largest potential hernial portal in the human body. Excessive distensibility of the hiatus is strongly associated with prolapse and prolapse recurrence. Distension occurs not just laterally but also caudally, resulting in perineal descent. This implies that the levator outlet, ie., the true plane of minimal dimensions, may not remain a 'Euclidean' (ie., flat) plane but may become warped. This phenomenon has been described in a study using MRI in a small number of patients (1).
