Wollondilly diabetes programme : consumer evaluation of a pilot patient passport template

Abstract

This study found that inner regional patients with diabetes find an electronic records system useful for integrating their diabetes"related care from different HCPs. Patients were satisfied with the platform's aims in improving care and found their profiles easy to access and navigate. The use of SwishCare will be continued in this setting and could be extended to other regional/rural communities.
