Abstract
Introduction: There have been a number of recent advances related to procedural techniques, TIPS stent technology, and emerging indications for TIPS. With this rapid evolution of knowledge, there remains lack of consensus on which patients should receive a TIPS in absence of dedicated guidance on TIPS referral pathways and practice guidelines in the Australian context. As a result, practices and outcomes vary greatly across institutions and significant knowledge gaps exist. This consensus guideline is the first guideline in Australia. It outlines 70 evidence-based practice recommendations and the detailed evidence underlying them. The recommendations are intended for use by healthcare professionals within Australia managing adult patients with portal hypertensive complications of liver disease who are considered for a TIPS implementation including pre-, peri-, and post-procedural care aspects. Methods and recommendations: This consensus statement has been developed by specialists in hepatology, interventional radiology, cardiology, haematology, primary care including both medical practitioners, nurses, and clinical researchers. The statement addresses four domains related to TIPS including: (1) Preparation for TIPS, patients’ selection, PreTIPS workup; (2) Best procedural practice; (3) Post operative care and follow up t; and (4) Indications. Two rounds of modified Delphi process were used to reach consensus on the 70 recommendations.Changes as a result of this consensus statement: Adoption of and adherence to the evidence-based recommendations in this consensus statement will reduce clinical variation. Ultimately, this should lead to lead to system level improvements in quality of care and outcomes for patients having TIPS implementation.
